Household Information

Fill out the form below with every adult on the lease and your household details. A case manager or family member is welcome to help.

Secure ID Information

Each adult completes a short, secure DocuSign form with their Social Security number, date of birth, and a photo of their driver's license or state ID.

Have ready: full legal names, email and mobile phone for each adult, an emergency contact, and pet and vehicle details if you have them.

Can't Fill It Out Online?

Please use the online form above first. It is the fastest way. Only if you cannot do it online, print both parts, fill them out, and bring them with your driver's license or state ID to our office at 3300 Clinton Parkway Court, Suite 110, Lawrence, KS 66047. Please hand them to our team in person. Never email or text a form that has a Social Security number on it.
